VOLUNTEER INSTRUCTIONS
Thank you for your interest in
becoming an Operation Our Town Volunteer. It is only through the
support of active community members that we will be able to take
back our neighborhoods from illegal drugs and crime.
1. Complete the
Operation Our Town “Volunteer Application Form” on-line at
www.operationourtown.org/http://www.operationourtown.com/volunteer_form.htm or contact our
office for a printed form that can be completed and mailed.

All Our Town volunteer
positions require both a “PA State Police Criminal History Record”
(Form SP-164, or Act 34 Clearance) and a “PA Child Abuse History Clearance” (Form CY-113,
or Act 151 Clearance).
· PA
State charges a $10.00 fee to process each of these clearances, for
a total cost of $20.00.
· The
request for a Criminal History Record can be
completed immediately on-line with a credit card by clicking on
the "Pennsylvania Access to Criminal History (PATCH) Web
Site" link on the PA State Police web-site at: www.portal.state.pa.us/portal/server.pt?open=512&objID
=4451&&PageID=458621&level=2&css=L2&mode=2.
The
application may also be printed from this site, completed and
mailed with payment to the PA State Police.
· It
is necessary to have a completed "Criminal History Record” to
submit with
the application for the “Child Abuse History Clearance.”
· The “Child Abuse History
Clearance” form can be downloaded at:

· Printed forms
to apply for each of these clearances are also available
through our office.

Upon receipt of
your processed clearances from PA State, submit copies to:
Operation Our Town, PO Box 2486, Altoona, PA 16603.
The majority of
our volunteers are contacted through e-mail. If you do not have
access to e-mail we will make every effort to contact you through
alternate methods as time permits. We also welcome your phone
inquiries regarding upcoming volunteer opportunities.
